I downloaded the app, but I can't login?

In order to login to our app, you must be registered with EZCareLink. Follow the Contact Us links on our website, email us at info@ezcarelink.com or call us to get started. Once you are registered, our office will email your login credentials and help with any troubleshooting.

What are the fees/costs involved?

Using the EZCareLink App is free for workers who register with us. Fees for Facilities using the EZCareLink App are minimal, inquire at info@ezcarelink.com.

What documentation do I need to provide to register on the EZCareLink App?

Caregivers:

  • Current CPR Certification
  • Current TB Test; either skin test or chest x-ray
  • Current Physical Exam
  • Complete Live Scan Fingerprinting (We will provide form for you to take to Live Scan, but Caregivers are responsible for paying any fees associated with fingerprinting. If you have fingerprint clearance through the Department of Justice completed within 6 months, we may accept this documentation.)
  • Complete Personal Data Registration Form
  • Home Health Aide Registry ID.
  • Provide copy of SSN Card and Driver’s License
  • Proof of Professional Liability Insurance

CNA, LVN or RN:

  • Current License
  • Current CPR Certification
  • Current TB Test; either skin test or chest x-ray
  • Current Physical Exam
  • Complete Personal Data Registration Form
  • Provide copy of SSN Card and Driver’s License
  • Proof of Professional Liability Insurance
